Requesting ELSA support for your child

If you feel your child may benefit from ELSA support, you can complete our Parent Referral Form using the form below. The form helps us understand what is happening for your child, what you would like to see improve, and any information that may help us plan the right support.

Once we receive the form, a member of our Family Support Team will contact you within two weeks to talk through your request and, where appropriate, agree the next steps with you and your child’s teacher.
